Early Cancellation Fee

By submitting this form, you accept the $180 early cancellation fee to your card on file.
Per contract, if you have a fixed term of 3 months and you decide to cancel your contract before the end of the term, we have a $180 contract cancellation policy. This fee will be charged to your card on file immediately after we cancel your membership. Any remaining appointments will be cancelled and your account will be flat.
This fee only applies to memberships with fixed term, so if you are on your last month of the term or currently on a month to month membership, this fee will not apply.
